Yirmi Yaş Dişleri

Sürme zamanı geldiği halde çene kemiği içinde kalarak sürmeyen dişlere gömük dişler adı verilir. Çoğunlukla yirmi yaş dişleri, nadir olarak köpek (kanin) dişleri ve çok nadir olarak da diğer dişler gömük olarak kalabilir.

20 yaş dişleri 17-35 yaşları arasında süren, ikinci azı dişlerinin arkasında yer alan sonuncu dişlerdir. Akıl dişleri olarak da adlandırılırlar. Bu dişlerin ağızda bırakılıp bırakılmama konusu halen tartışmalıdır. Anormal bir pozisyonda olup sürmeyen ve etrafındaki dokulara zarar vermeyen dişlerin çekilmesine gerek yoktur. Bu dişler çene kavisinin dar olması, sürme pozisyonunun yanlış yerde olması, kemiğin o bölgede yoğun olması gibi çeşitli diş sürme problemlerine bağlı olarak çoğunlukla çene kemiği içinde gömülü veya yarı gömülü biçimde kalmaktadırlar.

20 yaş dişleri en fazla sorun çıkaran dişlerdir. Gömülü kaldıkları için iltihaplanmalara, kimi zaman da ağrı gibi herhangi bir belirti dahi vermeden kistlere sebep olabilirler. Ayrıca diğer dişleri öne iterek, dişlerde eğrilik ve çapraşıklığa sebep olabilirler. Ağızda yarı sürmüş durumda bulunan 20 yaş dişleri ise diş fırçası ile tam temizlenemedikleri için çürümeye müsait dişlerdir. Çürük sonucu oluşan ağrı ve ağız kokusu nedeniylede bu dişlerin alınması gerekebilir. Kimi zaman da, hiçbir problem oluşturmadığı halde diş teli takan (ortodontik tedavi gören) veya protez tedavisi gören hastalarda, tedavinin ilerleyişini bozmaması için çekilmeleri gerekir. Bizler 20 yaş dişlerini birer “saatli bombaya” benzetiriz. Uzun bir süre sessiz kalıp aniden yüzde bir şişmeye, çenelerin kilitlenmesine ya da şiddetli bir ağrıya neden olabilirler. Bu nedenle bu tip dişlerin kontrol edilmesi ve gerekli ise çekilmesi gerekir.

Kötü pozisyonlu bir 20 yaş dişi, problem yaratsın ya da yaratmasın çekilmesi gerekir. Bu dişlerin çekimi hasta uyutulmadan, sadece ilgili bölgeyi uyuşturan bir iğne yapıldıktan sonra, küçük bir ameliyatla gerçekleştirilir. Zamanla gömük dişin çene kemiği ile kaynaşma ihtimali bulunduğu için ve yaş ilerledikçe iyileşme daha uzun süreceği için bu ameliyatın ileri yaşlarda yapılması daha zordur. Operasyon sonrası doktorunuzun vereceği uygun antibiyotik, ağrı kesici ve ağız gargarası gibi ilaçların düzgün kullanımı ile ortalama 1 hafta içinde operasyon bölgesi iyileşmektedir.
